All new patients are asked to complete the “Patient Intake Form” in order to be seen at a Cochlear Hearing Center. Please download the forms, review them and fill them out prior to arriving at our clinic for your first appointment. If you are unable to print and fill them out ahead of time, please arrive 20 minutes early to your first appointment.
